Saint Francis uses Magnus Health to collect all student medical information. Families should access their Magnus Account to complete mandatory health forms (listed as requirements in Magnus) and to update any important information for your student. Once you have accessed your Magnus account, please click the "complete now" button next to your student’s name in order to view the following required forms in your student's To Do list. These forms will be available on Thursday, May 8 and must be completed by Monday, May 19.

Please note that the Doctor Physical form for 10-12 grade students is no longer stored in Magnus Health, but is now stored in Home Campus, our Athletic Clearance System. The Doctor Physical Exam form is only required for 10-12 grade students who plan to participate in Athletics for the upcoming school year.
